What Is Care Management?

Our care management approach is client-centered, providing services that support individuals facing ongoing health challenges. Working with clients and their families, we use our expertise to navigate complex medical, legal, and insurance systems, helping to provide clear answers to questions and offering a choice of solutions during times of uncertainty. Our mission is to help reduce stress and worry for our clients and their families, and to minimize disruption to family members’ normal life needs, such as time off from work.

  • Medical management, including direct communication with providers, doctor visit coordination, medication management, and ongoing review for changes in condition, with direct reporting to family as needed

  • Recommending and overseeing outside resources, such as medical and non-medical home health agencies
